Agency Description: Agency serves residents of nursing/personal care homes and older adults in the community by providing the following programs; nursing home visitation, ombudsman (advocacy), volunteer ombudsman, guardianship and community services (bus transportation and P.E.E.R).
COMMUNITY SERVICES
Through the use of a 14 passenger, 4 wheelchair capable vehicle, residents of Lackawanna and Wayne Counties area nursing homes and personal care homes can maintain contact and active involvement in community activities. Coordinator researches, creates, schedules, plans and implements various programs. Programs include: community outings, dining, shopping trips, Scranton/Wilkes-Barre Yankees games, intergenerational activities with local day care centers, volunteer entertainment, pet therapy, etc. Benefits are social, therapeutic and educational.
GUARDIANSHIP PROGRAM
The Guardianship Services Program of Serving Seniors, Inc. was initiated in December of 1993 in an effort to respond to the needs of consumers/wards who have been found by the Court to be incapacitated and in need of a surrogate decision maker. Serving Seniors, Inc. acts as a Guardian of Person and/or Guardian of Estate when there is no other individual or institution willing, able, or suitable to act as Guardian.
Serving Seniors, Inc. accepts the responsibility for providing Guardianship of Person and/or Guardian of Estate services as ordered by the Court and in accordance with the 1992 Pennsylvania Guardianship Reform Legislation.
Serving Seniors, Inc. is staffed with skilled professionals. We are an active member with the National Guardianship Association, which establishes standards and ethics for individuals and organizations acting as court appointed guardians.
Serving Seniors, Inc. is committed to the provision of quality guardianship services to its clients and makes every effort to assist and support them in the development of maximum self-reliance and independence.
Who is Eligible for Guardian of Person and/or Estate Services?
Pennsylvania State residents eighteen (18) years of age or older.
Who can make a referral for Guardianship Services?
Referrals may be made by individuals within the community, hospitals, long-term living facilities, physicians, attorneys, the Court, financial institutions, and social service agencies.
OMBUDSMAN PROGRAM
The Ombudsman Program mandated by the Pennsylvania State Department of Aging, investigates, verifies and resolves complaints made by, or on behalf of, people sixty (60) years of age or older who are consumers of long term care. The Program Director ensures the understanding and implementation of residents’ rights as well as changes in legislation and other current events as they affect health and well-being of residents of Lackawanna County. Contact with the Ombudsman Program may be made anonymously and complete confidentiality is guaranteed. The program is free of charge and available to anyone regardless of income. The Ombudsman office may also be contacted for information regarding all aspects of long term care as well as for assistance with difficult placement.
NURSING HOME VISITATION
Provides visitation and advocacy services to area nursing home and personal care home residents by trained volunteers of all ages. Special events includes: intergenerational activities, reading program, pet therapy, the Golden Service Club, and Adopt-A-Resident program
VOLUNTEER OMBUDSMAN PROGRAM
The purpose of this program is to develop a systematic approach for utilizing volunteers in the delivery of Ombudsman services. The volunteers will serve as assistants to the Ombudsman by providing visitation, program education and eventually mild program solving.Volunteers must complete necessary training provided by the Ombudsman as well as the Pa. Dept. of Aging.
